Ochsner 65 Plus is designed exclusively for older patients. It offers personalized healthcare, social support, and wellness resources to care for all aspects of a person's well-being.

Patients benefit from a multidisciplinary team that collaborates to deliver customized care plans. It includes a primary care physician who provides extended one-on-one time, with appointments often lasting between 40 minutes and one hour. Care teams also include a licensed clinical social worker trained in behavioral therapy, a clinical pharmacist to help people manage even the most complex of medication regimens, a dietitian who can help provide nutritional counseling support, a health coach, a fitness instructor and others.

Specializing in common conditions experienced by older adults, like arthritis, diabetes and heart failure, Ochsner 65 Plus provides a full suite of services, including labs, fitness centers, immunizations, infusions and physical therapy. Furthermore, its patients are seamlessly connected to the hundreds of specialties and resources provided throughout the Ochsner Health system.

The offerings extend beyond traditional medical services to include social and wellness opportunities matched to patients’ interests and lifestyles, such as group fitness classes, cooking demonstrations, health coaching, and card games, among many others.

Ochsner 65 Plus staff also help patients obtain difficult-to-acquire medications, navigate insurance, obtain financial assistance, and utilize Ochsner Digital Medicine technologies.

“Ochsner 65 Plus clinics are free-standing primary care clinics that are designed specifically to help meet the unique needs of older adults, and where we take a comprehensive approach to primary care with the patient’s unique health needs in mind,” said Dr. Kenny Cole, Ochsner’s system vice president for clinical improvement. “Along with our specialized physicians, we have built a team around the patient that provides customized care based on their personal goals and preferences.”
