Senior Vice President, Population Health and Chief Executive Officer, Ochsner Health Plan

As Senior Vice President, Population Health and Chief Executive Officer, Ochsner Health Plan, Jeff Fernandez seeks creative and innovative ways to improve community wellness, promote value-based care models and redefine primary care to drive positive outcomes.

In his newly created position, Jeff will oversee the growth of the Ochsner Health Network, Louisiana’s largest clinically integrated network, continuing its successful focus on risk-based and value-based care delivery models, and the Ochsner Accountable Care Network, an Enhanced Track Medicare Shared Savings Program (MSSP) ACO, which manages over 40,000 Medicare lives.

His responsibilities also included leading their nationwide MarketPoint sales organization, where he was responsible for external, career and telesales distribution for Humana’s Medicare Advantage products.

Fernandez received a bachelor’s degree in marketing from LSU, an MBA from the University of New Orleans, and then went on to achieve a law degree from Loyola University New Orleans College of Law.
